Contest 1: Brief Me on HPV: "Artwork and Slogan"
WIN $500
- Design artwork and a slogan relating to HPV suitable for printing on men’s or women’s garments (t-shirts, tank tops, shorts, sweatshirts, etc)
- Promote Your Design! To generate votes, you can promote your design and logo using any social networking websites and encouraging your friends, family, and followers to vote for you. Voting opens on October 1, 2010 and closes on October 17, 2010.
Contest 2: Brief Me on HPV "Refer-A-Friend"
WIN $75
- Tell your friends! Encourage your friends to like our Facebook page. Four winners will be randomly chosen to win a $75 gift card.
The winner of contest #1 is NOT eligible to win contest #2. The winners of the "Artwork and Slogan" contest and the “Refer-a-Friend” prize must be different individuals.
HOW TO ENTER: Go to http://hpvbrief.org/participate and enter the contest by uploading your image. All entries must include original artwork and a slogan aimed at promoting awareness and educating the public about HPV and/or the HPV vaccine series. Entries should be designed to appeal to an audience between the ages of 18 and 26. You may enter as many unique entries as you want.
Be original! Copy-catting is not cool!
AGE AND OTHER ELIGIBILITY: People ages 13 and over may submit their design(s) or spread the word about the contest. BUT only California residents ages 18 and over are eligible to win.
PRIZES:
Grand Prize of $500
One winner selected by a combination of public vote and a committee decision will win $500. To generate votes, you can promote your design and logo using any social networking website and encourage your friends, family, and followers to vote for you. In addition, the winner will receive an item of clothing of their choice with their design printed on it. The winning design will be featured on our website.

I hereby irrevocably consent to and authorize the use, re-use, publish and republish, and to license the right to use, re-use, publish and republish my entry in its sole discretion, in any medium or form of distribution without restriction as to changes or transformation in conjunction with my own or a fictitious name, made through any and all media now or hereafter known for any purposes whatsoever, including, without limitation, illustration, art, promotion, advertising or trade, by the State of California Department of Public Health (CDPH), or anyone authorized by CDPH, of any and all logos and/or slogan, and digital images submitted by me and/or my child, for any purpose whatsoever, without compensation to me or my child. All photographic prints, including digital photos, and/or digitized computer outputs shall constitute CDPH property, solely and completely. I understand this submitted image and/or photo is intended for health promotion purposes, including display on the hpvbrief.org website, public service advertising, posters, displays, audio-video presentations, and other promotional, training, educational materials, and for usage on the Internet, including social networking websites. I hereby release, discharge and agree to save harmless California Department of Public Health, and its employees or agents, affiliates, legal representatives or assigns and all persons acting under its permission or upon its authority or for whom it is acting, from any liability by virtue of any publication of my likeness or testimonial, including, without limitation, claims for libel or invasion of privacy, as well as any liability arising by virtue of any blurring distortion, alteration, optical illusion of use in composite form, whether intentional or otherwise, that may occur or be produced in the making of such picture or recording(s) or in any processing tending towards the completion of the finished product. I hereby warrant that I am either of legal age or have permission from a parent or legal guardian to submit material to this site and have every right to contract in my own name in the above regard. I understand that if I request the removal of my own or my child’s submitted picture, then the webmaster will do so promptly. I state FURTHER that I have read the above AUTHORIZATION and release prior to its execution, and that I am fully familiar with the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.
Entries that are lewd, obscene, pornographic, disparaging of Sponsor or affiliated entities or otherwise contain objectionable material WILL be disqualified and removed from the campaign.
